Wasn't there an early Graham Parker album like that, too -- The Parkerilla or something, with a 12-inch single of Don't Ask Me Questions on side 4?
Funkadelic One Nation Under a Groove came with a 7-inch inside that was CALLED Side C, but I don't know if that counts, either.
Also, I have an M "Pop Muzik" single which is a 3-sided single -- on one side, there are two different concentric grooves, so depending on where you put the needle down, it plays either Pop Muzik or M Factor.

I think it's more a mastering/sound quality concern than any label's conscious artistic decision but i could be wrong.

yeah but there isn't going to be any noticeable difference in sound quality between a side of music lasting 13 mins and one lasting 17 mins.
a couple of examples I can think of:
Coil, Musick to Play in the Dark vol. 2 – has a v. nice etching on side 4Death in June, Something is Coming
also I'm pretty sure I have a Nurse With Wound LP like this w/etching on side 4 but I can't remember which one
