It's Canada-wide, and it's a rebate for the Goods and Services Tax (GST) as mentioned above. It's set up so that the more you earn, the more you get back up to a certain point, under the assumption that you're spending more on goods and services, and therefore deserve a bigger rebate; at about $30,000/year though, it just stops arriving at all because apparently you've made it! Yay! I think it tops out at something like $75 quarterly.

There's a checkbox that you have to tick off when you submit your Income Tax, authorizing the government to give you GST rebates if you qualify. If you don't tick it off, you don't get the money.

if you're an American and you spend more than $300 on certain goods in Canada, you can get a GST rebate too.
