People who are very nice but have tiny little habits that they probably do without realising it. Foor instance, my boss is actually a fairly good bloke, but he has this annoying habit of clearing his throat EVERY 10 SECONDS!
I didn't even notice this until I went on a business trip with him that involved a six hour drive there and back and by the end I was ready to throttle him. It would often start with a low guttural hum as if he's singing a tune to himself but withott a melody. This he would do for about 5-10 seconds getting a bit louder and a bit louder and then climax in a "aaUUhhhhhGGHHMM!".
Maybe there's a genuine reason for this or it's a nervous twitch. Either way it drives me up the wall.

Shit, I do this a lot. I'm driving MYSELF insane let alone anyone else. I've had it investigated and it was thought to be sinus related, then some precursor to asthma, then chronic rhinitis and sundry other things. It doesn't appear to be any of these but I'm still left with a constant need to clear my throat or my voice goes funny or I can't breathe properly.

Well, maybe I shouldn't be so harsh as it probably is a problem he can't help. I myself am guilty of being a sniffer and I also sneeze a bit more than usual which I'm sure is extremely irritating, but I can't help this either.

Hahah, you're a tapper! I have a friend who does that except he's a death metal fan so it becomes more like a high speed slam. Going down the pub can be a severe headache and he's been shouted at by the landlady on numerous occasions!

my dad does the constant throat-clearing thing. you tend to not even notice it but when you do it's ALL you can notice.

my best friend is a nervous leg shaker. sometimes he does it so vigorously that it makes my seat vibrate when we're in a car together.

I sometimes do that if I'm sitting awkwardly and the balance on my foot and leg is just slightly off. At least I think that's what it might be. Only happens once in a blue moon though.
