I got a letter from the U.S. District Court for an approved refund notice. (from U.S. District Court Settlement Administrator in Philly, PA, in case someone here wants to question its veracity.) I can choose ONE OF THREE FORMS:
THE GREEN FORM: $25. You do nothing except sign then take the $25 and run, motherfucker.
THE BLUE FORM: Give an estimation how many days between 1996 and 2006 that you were outside the U.S., state your frequency of purpose for travel, then sign and you're eligible for up to 1% of your foreign transactions.
THE RED FORM: This is for people who travelled a lot and have their bookkeeping i.e. shit together. For every year between 1996 and 2006, state up to two cards/accounts and estimate how much money in US dollars was done via foreign transactions. Then you are possibly eligible for up to 3% of those transactions.

REFUND OPTION 2: Request a *Total Estimation Refund* based on typical spending during travel and your answers to a few questions about your own travel outside of the U.S. This Option is recommended if you traveled outside of the U.S. for more than one week or had foreign transactions of more than $2,500 using your eligible cards during the 1996 to 2006 period. Refunds will be the maximum of 1% of estimated foreign transactions. (Blue Form); OR
REFUND OPTION 3: Request a refund based on information that you provide concerning your *Annual Estimated* foreign transactions during the 1996 to 2006 period. This Option is recommended if you had extensive foreign travel or foreign transactions and are willing to provide year-by-year information. Refunds will be a maximum of 1% to 3% of foreign transactions. This is the only Option you can use to get a refund for corporate card use. (Red Form)
Enclosed are three forms, one for each Refund Option. You may also file online at the Settlement Administrator's website www.ccfsettlement.com, using your Refund ID on the top of this letter. Please note that if the volume of claims is unexpectedly high, it may be necessary to adjust refund amounts.

If you get the settlement form in the mail, you won't need your card number. I just filled out my quick $25 form online and all I had to enter was a code # at the top of my letter.

filled this out today; any idea how they're going to estimate how much money you get if you fill out the second (blue) form? the green form assumes you were out for a week...so is it foolish to hope for four times the $25 if you were out for four weeks? yeah probably.
