looking at the cover? first bar? months?

Depends, really. Has very little to do with the cover, generally (as if that were the case, I certainly never would've gone near several albums which I love....great records sometimes come sleeved in hideous designs). Sometimes, it's immediate based on the sound, the production or even something as simple as a riff or a vocal hook. Not an exact science for me.

Yeah - a lot of the stuff is my like is very immediate; a lot more takes a little more time. MP3s are hugely useful here - I listen to MP3s on random most of the day and so I get the repeat-exposure certain tunes need.

Hearing something in a club can be the tipping point, too.

"Hearing something in a club can be the tipping point, too."

Good point, Tom. Sometimes, the context in which you first hear at track becomes intertwined with one's enjoyment of a track (or lack thereof).

have you had tastes/opinions changed in this way?

Go to bed, me.

Gaz - yes "Love Story" by Layo and Bushwacka grrr. (not in a club tho)

Changed? Not in that particular capactiy, I wouldn't think. If I'm in an environment that -- for one reason or another -- I'm enjoying or consider an environment condusive to new and interesting things and hear something, I'm probably more likely to have an open mind -- or even positive bias -- towards it. Conversely, if I'm having a dreadful time somewhere and hear a song, it's not that I'm immediately going to think that song is shit, but I'll be less inclined to give it a shot (as I'll probably mentally associate the music with the unpleasant situation).

it is too early to say

With some exceptions, it takes me quite a while. Which makes me wonder how many times someone like Robert Christgau needs to listen to something before he reviews it. Six-eight times wouldn't be enough for me.

Almost always immediately.

It didn't even take an entire measure with some stuff (System of a Down, Ani DiFranco, Amon Tobin for example). I got hooked pretty instantly on most of my favorites.

However, I will say that it took listening-to-him-on-acid for me to fully "get" Sun Ra...but when I did, jinkees!
